Gain knowledge about the operationalization of survivorship care plans across diverse survivorship models. You’ll hear from three panelists, each providing their unique perspective on care plan implementation and overcoming barriers to developing or delivering survivorship care plans. You’ll also have the chance to participate in a group discussion with the panelists and other attendees to share your own insight.

Objectives

1. Describe three examples of survivorship care plan implementation.
2. Identify at least one technique for survivorship care plan implementation that would be helpful in their own setting.
3. Discuss at least one strategy for overcoming a common barrier to developing or delivering survivorship care plans.
